Occasionally a student will encounter an issue with an online course that he or she doesn’t know how to resolve. When this occurs, students should always contact the instructor first. Most issues are resolved when a student communicates their frustrations/concerns with the instructor. Students are encouraged to review the appropriate guidelines for each WVU campus. WVU Online
Only students enrolled in a WVU Online major/program or course should complete this form. Students not enrolled in a WVU Online program should visit The Office of the Provost site and complete the proper form.

The student should begin the complaint process with the institution and if resolution is not found, the student would contact the Institution’s Home State SARA Portal Entity.
